Managing your information

Use “Clear this form” to remove answers from the current screen. Old personal-data cookies and localStorage created by the previous version are removed when a funnel loads. Clearing the screen does not delete draft answers already saved to Google Sheets. Use the request form below to ask for deletion. Local synced drafts expire after 24 hours of inactivity when cleanup runs.

For submitted requests, contact the operator to ask about correction, deletion, access, or stopping further contact. The operator must also handle copies in its Google Sheet and any systems it uses outside this application.

Do Not Sell or Share My Personal Information

The current application does not sell personal information or share it for cross-context behavioral advertising. You may submit an opt-out request below for the operator to review. This form also accepts access, correction, deletion, contact preferences, and appeals. Submitting a request records it in a private inbox; processing requires the operator's review and may require identity verification. It does not automatically remove information from Google Sheets or change external systems.

Include only what we need to locate your request. Do not include passwords, payment details, or identity documents. We may need to verify your identity before changing or releasing information.

Contact requests are kept in a private inbox until handled. When marked resolved, the message and email are removed; reference metadata is kept for up to 30 days. This form does not automatically delete quote records or unsubscribe you from external services.
